Care Coordination for Maternal Health – Birth | Guide Overview

Care Coordination for Maternal Health – Birth applies the DirectTrust Metadata and Payloads (MAP) Framework to information sharing across pregnancy, birth, and postpartum care. This guide describes the use case and composes reusable transactions to support its workflows.

Guide Overview

The MAP Use Case Framework connects the Information Exchange Objectives in Chapter 1 with the responsible parties, capacities, business roles, and Transaction Participation Roles described in Chapter 2. It organizes how those participants compose reusable transactions into workflows to accomplish the objectives. The MAP Reusable Transaction Catalog (MAP RTC) identifies the Shared Activities available for reuse and connects each transaction to its defining specification. Chapter 2 shows how the selected transactions support this use case through the Objective-to-Transaction Matrix and Actor Transaction Diagrams.

The MAP Transaction Framework in Chapter 3 provides the common structure and requirements used to define transactions, including information objects, structured payload metadata, Technical Actor responsibilities, routing, workflow status, and Provenance. Each Transaction Definition applies these requirements to a Shared Activity. Use-case-specific requirements explain how a reusable transaction supports the maternal-health workflow.

Chapter 4 describes Capability Statements and endpoint discovery requirements that help participants identify systems able to support the required transactions and select an appropriate Routing Target. Chapter 5 provides General Implementer Guidance for applying the use-case and transaction requirements together, including the use of the Vocabulary Package, conformance artifacts, examples, and supporting appendices.
