D.3 SubmissionSet to FHIR List Mapping
SubmissionSet-to-FHIR List mapping inventory.
D.3.1 Mapping Principles and Governing Profiles
The MAP SubmissionSet profile derives directly from the FHIR R4 List. IHE MHD Minimal SubmissionSet is a compatibility reference, not the mandatory baseDefinition. One MAP List profile permits List.mode=working or List.mode=snapshot through a Required binding to MAPSubmissionSetListModeVS. The current transaction SubmissionSet uses working; when the applicable Transaction Definition calls for an identification-only original SubmissionSet snapshot, that separate List uses snapshot. The mode is consistent with FHIR List maintenance semantics and does not, by itself, universally define transaction role. MAP Chapter 3 and applicable Transaction Definitions govern conditional metadata and membership requirements.
D.3.2 Standard SubmissionSet Metadata Mapping
| XD field | FHIR candidate or mapping | Status |
|---|---|---|
| entryUUID | List.identifier (typed entryUUID) | 1..1; do not use List.id |
| uniqueId | List.identifier (typed uniqueId) | 1..1; Shared Activity identity |
| sourceId | List.extension ihe-sourceId | 1..1; contributor Organization Identity OID |
| submissionTime | List.date | 1..1; package assembly time |
| contentTypeCode | List.extension ihe-designationType | 0..1 RE; preserve classification |
| purpose | List.extension map-submission-purpose.valueCodeableConcept | 1..1; Extensible TransactionTypesVS |
| patientId | List.subject → Patient.identifier | 0..1 conditional; exchange-domain ID only |
| sourcePatientId | List.extension map-submissionset-sourcePatientId[*] | 0..* conditional; Identifier plus original CX when available |
| sourcePatientInfo | List.extension map-submissionset-sourcePatientInfo | 0..1 set; repeated PID fields preserved |
| referenceIdList | List.extension map-submissionset-referenceIdList[*] | 0..*; typed Identifier plus optional original CXi |
| author | List.source; IHE ihe-authorOrg where applicable | 1..1 author; preserve author classification details |
| author.authorPerson | List.source Practitioner/PractitionerRole as appropriate | 0..1 per author; XCN may require preservation |
| author.authorInstitution | List.source Organization or ihe-authorOrg | 1..1 per author; XON/authority preserved |
| author.authorRole | PractitionerRole.code or additional author metadata | 0..*; preserve author role code |
| author.authorSpecialty | PractitionerRole.specialty or additional author metadata | 0..*; preserve coding |
| author.authorTelecommunication | Practitioner/Organization telecom or preservation metadata | 0..*; XTN/Direct semantics retained |
| intendedRecipient | List.extension ihe-intendedRecipient | 0..*; not transport intermediaries |
| title | List.title | 0..1 |
| comments | List.note.text | 0..1 |
| availabilityStatus | List.status = current for XD Approved | 0..1; package status not workflow outcome |
| homeCommunityId | MAP preservation extension on List when supplied | 0..1; established community identifier |
| limitedMetadata | MAP preservation extension on List when supplied | 0..1; classification flag, not missing-field indicator |
| List.mode (MAP packaging discriminator) | List.mode | 1..1; working for current transaction SubmissionSet; snapshot for original identification-only List when required by Transaction Definition; Required MAPSubmissionSetListModeVS binding. |
D.3.3 MAP-defined SubmissionSet Extension Mappings
| Extension | XD Slot | FHIR mapping status |
|---|---|---|
| purpose | urn:dt-org:dsm:map:purpose | map-submission-purpose (CodeableConcept; Extensible TransactionTypesVS; 1..1) |
| sourcePatientId | urn:dt-org:dsm:map:sourcePatientId | map-submissionset-sourcePatientId; 0..*; Identifier plus optional original XD CX |
| sourcePatientInfo | urn:dt-org:dsm:map:sourcePatientInfo | map-submissionset-sourcePatientInfo; 0..1 set with repeated verbatim PID fields |
| referenceIdList | urn:dt-org:dsm:map:referenceIdList | map-submissionset-referenceIdList; 0..*; Identifier + Context TypeCode Extensible DTContextTypeCodeVS + optional original XD CXi |
D.3.4 SubmissionSet Membership and Identifier Relationships
For the current transaction SubmissionSet (working), List.entry.item references each submitted information object DocumentReference. Bundle.entry alone does not establish SubmissionSet membership. For an original SubmissionSet identification snapshot (snapshot), List.entry MAY be absent and SHALL NOT be interpreted as a complete reproduction of original package membership. List.id is a FHIR logical identifier and is not a substitute for the SubmissionSet uniqueId or entryUUID metadata.
Workflow-status interpretation. In send-workflow-status, the current transaction SubmissionSet (working) identifies the submitted report objects. A distinct original SubmissionSet identification snapshot (snapshot) is the Primary Information Object and carries the original SubmissionSet.uniqueId, purpose, sourcePatientId and intendedRecipient for correlation. The four FHIR mappings are respectively typed List.identifier, List.extension[map-submission-purpose].valueCodeableConcept, List.extension[map-submissionset-sourcePatientId], and List.extension[ihe-intendedRecipient]. List.subject identifies exchange-domain patientId and SHALL NOT be substituted for sourcePatientId. The applicable send-workflow-status GenTXDef governs correlation, handling of legitimately absent identifiers, and administrative anomalies.
D.3.5 Conformance and Mapping Constraints
MAP SubmissionSet FHIR mapping is specified by the tables above and accompanying extension definitions. The single MAP List profile permits working and snapshot; the original identification-only snapshot need not reproduce List.entry membership. Current transaction SubmissionSet membership remains mandatory under D.2. PatientId and sourcePatientId are distinct metadata elements; List.subject represents the exchange-domain patientId and List.extension[map-submissionset-sourcePatientId] preserves the originating source patient identifiers. Transaction-specific correlation requirements are governed by the applicable GenTXDef.
